TOP 10 CASUALTY CHARACTERS OF ALL TIME! WHO MADE THE LIST?

Casualty has seen hundreds of characters come and go over its incredible 36-year run. From heroic paramedics to doctors facing personal challenges, this show has given us some of the most iconic and unforgettable characters in TV history. Today, I’m counting down my personal Top 10 Casualty Characters—a ranking that’s shaped by my experiences since 2007. Remember, this list is personal, so if your favorite isn’t on it, don’t worry! 10. Ruby Sparks

Ruby Sparks joined the show in 2018 as a by-the-book paramedic who wasn’t afraid to follow the rules. In a world of heroic, risk-taking paramedics, Ruby was a breath of fresh air. Played by Maddy Hill, her development from a rule-follower to a caring guardian figure for her niece was a compelling arc. Her exit in 2020, where she left to be a mother, was a poignant ending for a character who had so much potential. Despite her short time on the show, Ruby made a lasting impression.

9. Tess Bateman

Tess Bateman was the motherly figure of the ED. Her character wasn’t always the most dramatic, but Tess was a reliable and strong presence. Whether giving advice to her colleagues or protecting her nurses, Tess always exuded warmth and authority. She might not have had the flashiest storylines, but her comforting demeanor made her a beloved character.  Casualty spoilers (January 4)

8. Adam Truman

Adam Truman had it all—charisma, leadership, and dedication. However, beneath his polished exterior, Adam faced deep emotional turmoil. From the tragic loss of his son to the collapse of his relationship with Jessica, Adam’s descent into depression was heartbreaking. His exit from Casualty, after a heroic act in an airport collapse, was one of the show’s most memorable moments, and he left behind an incredible legacy.

7. Alicia Monroe

Alicia Monroe began as a cheeky, immature junior doctor, but her character evolved dramatically over the years. Her relationship with Ethan was one of the highlights of the show, and her storyline surrounding her rape was handled with sensitivity and powerful performances from Chelsea Halfpenny. Alicia’s departure marked a bittersweet end to her journey, with her accepting that she deserved happiness even after facing such trauma.

6. Charlie Fairhead

Charlie Fairhead, the only character to appear continuously since the show began in 1986, is undoubtedly one of Casualty’s most iconic figures. As the steady, dependable presence in the ED, Charlie has always been there for his team, offering advice and guidance. 5. Jeff Collier

Jeff Collier is the quintessential hero paramedic. Fearless and willing to risk it all to save lives, Jeff was always the first to jump into danger. His heroic antics, like rescuing people from burning buildings and dangerous cliffside rescues, were thrilling to watch. His bond with Dixie was equally important, and Jeff’s shocking death in a minibus explosion remains one of the most tragic and memorable moments in Casualty history.

4. Zoe Hannah

Zoe Hannah was a strong and complex character who overcame many challenges in her career. From her rocky relationship with Nick Jordan to taking on the role of clinical lead after his departure, Zoe’s storylines were always emotionally engaging. Her relationship with Max Walker, despite their age gap, was one of the most touching aspects of her character. Her departure, where she left with Max for a fresh start, was a well-earned happy ending.

3. Ethan Hardy

Ethan Hardy’s journey on Casualty has been full of highs and lows. From his early days as a talented consultant to facing the devastating news of his Huntington’s diagnosis, Ethan’s emotional journey was one that resonated with fans. His grief after his brother’s death and his eventual, peaceful exit—leaving to raise his son—was a well-rounded conclusion to his story. Ethan became one of the most beloved characters on the show, and his departure was heartfelt.

2. Ruth Winters

Ruth Winters was one of the most complex characters Casualty has ever seen. Initially aloof and often at odds with her colleagues, Ruth’s character evolved dramatically over her time in the ED. Georgia Taylor’s portrayal of Ruth, especially in her mental health storyline, was outstanding. Ruth’s journey through bipolar disorder and her struggle with vulnerability was a powerful storyline. Her exit, where she found happiness with Jay and left for a new life, was a beautiful and well-deserved conclusion to her arc.

1. Dylan Keogh

There was no doubt in my mind—Dylan Keogh is Casualty’s greatest character. His dry wit, intense sarcasm, and emotional depth make him unforgettable. As the rebellious, maverick consultant, Dylan was always the character you couldn’t help but watch. His battle with OCD and his struggles with alcoholism were compelling, and his time as clinical lead showed a side of him that wasn’t just about rebellion but about caring for his team. Dylan Keogh’s longevity and William Beck’s exceptional portrayal have made him the ultimate Casualty icon.
